Immunization Request Forms
Your child will not be able to attend school after September 29, 2017 until the required immunizations are brought up to date and proof of the immunizations are presented to the nurse’s office. If you child is exempt from the immunizations for medical or conscientious reasons, please provide the original document to the nurse’s office immediately. These absences will be unexcused.
